For new students:

- Take the Assessment Test and interview.

3. What are the requirements needed for admission?
Initial requirements:
- Properly filled-out application form with 1X1 I.D. picture
- Photocopy of report card for incoming grades 1-9
Additional requirements after taking the Assessment
- New student
- -Photocopy of PSA Birth Certificate
- Medical Certificate (certifying that the child is physically fit to handle school work
(all documents must be placed in a long folder)
- for transferees
- Original report card
- Certificate of Good Moral Character for Grades 4-10
- For returning students
- Form 138 (Report card) and Form 137 (Permanent Transcript of Record)
4. How much are the books cost?
Book amount is available upon request.
5. How to request for a certain school forms and other school requirements needed?
Fill-out the request form in the office. Pay for the processing fee and wait for 3 to 5 working days.
6. What is ESC Scholarship?
Educational Service Contracting (ESC) is a government subsidy which entitles qualified grantees to annual financial assistance of 13k throughout Junior High School provided they meet the requirements for regular promotion in each grade level.
7. Who are eligible to avail the ESC?
- Elementary graduate from Public or Private DepEd Recognized School
- Incoming Grade 7 student (ALS, PEPT Grade 7 Qualifiers are included)
- Has not been a recipient of the ESC in the previous school years
- Filipino Citizen
8. What are the requirements needed for ESC?
- Properly filled-out ESC Form (available in the SRMS Office)
- Recent 2×2 ID Pictures (2copies)
- Photocopy of PSA Birth certificate
- Photo copy of Grade 6 Report Card
- Good Moral Certificate
- F137 from the previous school
- Latest Income Tax Return for the previous year or Certificate of Tax/ Exemption or Municipal Certification of Unemployment of Parents/ Legal Guardian
submit it to the SRMS Office for evaluation.

9. What other discounts/scholarships that can be availed by the students and what is the requirement needed to avail it?
Sibling Discount
- 2ND CHILD: less Php 500.00 on basic tuition fee
- 3RD CHILD: 50% discount on basic tuition fee
- 4TH CHILD: 100% discount on basic tuition fee
*Discount is applied to the youngest child
Referral Discount
Get Php 500.00 discount for every referral
(Referral form is available at the office)
Alumni Discount
10% discount on basic tuition fee for the child/ren of the bona fide SRMS Alumni (Must present a proof i.e. Diploma)

For unprivileged but deserving student
