Initial requirements
- Properly filled-out application form with 1X1 I.D. picture
- Photocopy of report card for incoming grades 1-9
Additional requirements after taking the Assessment
- New student
- -Photocopy of PSA Birth Certificate
- Medical Certificate ( certifying that the child is physically fit to handle school work
(all documents must be placed in a long folder)
- for transferees
- Original report card
- Certificate of Good Moral Character for Grades 4-10
- For returning students
- Form 138 ( Report card) and Form 137 (Permanent Transcript of Record)
